Residential Support Services

Adult Outreach Services

An individualized support network is available for adults with developmental disabilities who are living independently in the community. This support program primarily delivers services in the evening hours to accommodate individuals who are either employed or involved with other programs and activities during the day. If required, support is available during the day.

Individuals are presented with opportunities and supports to maintain and/or enhance their skills in:

  • Meal planning and food preparation
  • Shopping
  • Life skills
  • Health care
  • Decision making, personal finances and home management
  • Personal awareness, relationships, and community awareness and resources
  • Home safety
  • Recreation and leisure
  • Legal issues - ranging from assistance to the completion and understanding of documentation

Community resources are utilized where and when possible to assist in delivering support to individuals - maximizing their independence in their chosen lifestyles. Supports are both class style and one-on-one - this is entirely dependent on a person's needs. Supports are also available through a periodic contract of one to 20 hours per week.

Basic Admission Criteria:

  • A primary diagnosis of developmental disability.
  • 18years of age and over.
  • An application and complete medical are required.
  • A willingness to participate in an individual service plan and program goals.
  • Applicants must attend an intake interview.
  • Applicants must also have the desire to live independently and meet the basic safety criteria.

Priority is given to applicants within the Northeast region. All referral applications must be submitted to the St. Paul Abilities Network's residential director and be approved by the Persons with Developmental Disabilities Consultation Unit.

All personal living and transportation expenses are the individual's responsibility.
