Residential Support Services

Individual Residence Support Services

The Purpose: To provide individual home living supports unique to the needs of each person with disabilities, which will maximize their personal growth and independence.

The Program: Support is totally dependent on the individual situation, funding available and service plan objectives for each individual. (Community inclusion supports may range from minimal hours for an individual in an independent living situation through to 24-hour group home living situations for someone with high needs).

Presently the St. Paul Abilities Network delivers this form of residential services in many unique different living situations and predicts an increase for this type of service delivery in the future.

Basic Admission Criteria: Must be eligible and approved for Individualized Funding. Funding must be adequate to deliver the service and related costs. Must have own/shared furnishings. An application and complete medical has been completed and received. Participate in an individual service plan and personal goals.

Referral Procedure: All referral applications must be submitted the Residential Director and approved by the Northeast Persons with Developmental Consultation Unit. Special Note: The Director of Residential Support Services is available to assist with the development of the application for service and funding.

Expenses: Dependent on the needs, costs, funding structure and service plan for each individual and their situation.

Note: People often have their own homes or share a home with another person(s). The individual in service takes care of the typical costs of the home.
