Residential Support Services

Residence #3


This residence offers a supported residential environment for aging adults with developmental disabilities while providing the security and comforts of a permanent home.

Residents are presented with opportunities and supports to maintain and/or enhance their quality of:

  • Home living skills
  • Health care
  • Family and personal relationships
  • Recreation, leisure, and community involvement and awareness.
  • Planning and preparation for their current or future retirement lifestyle

Basic Admission Criteria:

  • A primary diagnosis of developmental disability.
  • 18 years of age or over. It is generally accepted that this home is for people who are 40 years of age or older.
  • An application and complete medical are required; both must be concluded and submitted before an admission can be processed. Residential staff and/or community resources can deal with medical management.
  • A willingness to participate in an individual service plan and program goals. Arrangements for day programs are necessary or plans for appropriate supports, including family approval, must be in place.
  • A visit to the residence, and attendance at an intake interview is required.
  • Applicants must have their own bedroom furnishings and personal effects.

Priority is given to applicants within the Northeast region. If an applicant is accepted as a resident and space is available, arrangements will be made for the individual to move in for a 90-day trial period.

The Home

The house is a duplex-style home with three bedrooms per side. It is fully accessible on the main floor, with wheelchair accessible showers and bathrooms, a century tub with lift, and includes all main floor facilities. Future plans include creating two smaller living units of three people each, out of the one larger home. The home is staffed 365 days per year: 18 hours per day Monday to Friday and 24 hours per day on weekends. Room and board is $19.00 per day.
