Family Outreach Services
Purpose :
To provide non-facility based supports for families who have a
child/children with identifiable disabilities.
Program :
Working in partnership with families, their individual needs are
assessed and supports are provided within the family home or community.
The Family Outreach Worker can provide information on disabilities,
teaching strategies for skill acquisition and a link to other service
providers.
Coordination of services, advocacy for service needs, support
plans, short term space/relief to the family can also be provided
through this program in partnership with St. Paul and Area Family
Wellness and Advisory Committee (SPAFWAC).
A Family Support Group meets monthly where families can meet,
support and share experiences with other families.
A lending library consisting of books, videos and resource materials
are available to families.
Basic Admission Criteria :
Identifiable disability.
31/2 - 18 years of age.
Family is seeking support and willing to participate with the
program.
Referral Procedure :
Self referral may be made by contacting the Family Support Worker
at (780)645-3339 or Family Services Coordinator at (780)645-3441.
Once a referral has been received, contact will be made with the
family to discuss service needs and options.
Expenses :
This service is funded through the Commission of Services to Children.
There are no additional service fees.
